All Pitt-FLEX forms due May 25

All employees must return their Pitt- FLEX open enrollment forms to the Human Resources benefits office, 200 Craig Hall, by May 25.

The benefits forms cover life, accidental death and dismemberment, and dependent term life insurances; medical and prescription insurances; vision insurance; dental insurance, and flexible spending account options.

All employees covered by Pitt's benefits plans, including those who are retaining their current benefits package, must fill out the form.

Rates for all benefits options are the same as last year's, except for optional life insurance rates, which are higher.

Choices made during the open enrollment period stay in effect for the duration of the plan year, July 1, 2001 to June 30, 2002, unless there is a change in family status (such as change in marital status or birth of a child). To qualify for a family status change, a Certification of Change in Family Status form must be submitted to Human Resources within 60 days of the event prompting the change in status.

Open enrollment forms and a pre-addressed return envelope were provided in the employee benefits enrollment packets. In the event of a lost form or if the subscriber makes a mistake in filling out the form, additional forms can be printed out from the open enrollment web site (openenroll.hr.pitt.edu).

Confirmation statements reflecting employees' choices are expected to be mailed to employees by June 25.

Human Resources is sponsoring a series of benefits fairs to supplement the information provided in the open enrollment guide and open enrollment choices booklet mailed to employees in March.

Remaining fairs on the Pittsburgh campus will be held May 8, 7th floor Art Gallery, Masonic Temple, and May 10, William Pitt Union Ballroom. Both fairs are noon – 2 p.m.

A benefits fair on the Johnstown campus is scheduled for May 9, with time and location to be announced.
